Many times when a 2x4 wall is indicated on a blue print a more aware builder would notice that a 3" pipe would be better served by a 2x6 wall and make the proper changes. Webit is tight but you can fit a 3"stack into 2x4 walls but you have to add 3/4" strapping to get the wall thickness to 4 1/4". as plumbfitter said you can't run any horizontal water lines across the stack unless you have 2x6. you don't need 4" inside the walls unless you are draining more than 3 toilets. at least in mass you put 3 toilets on 3"
